• A Grant Award Notification Form is required for every award of $1,000 or more
• The Additional Funds Form is for follow-on funding from the same funder
• An A to Z Grant Award Notification Form [xls] is required for A to Z grants from DPS Foundation
• Please follow the District Grants Procedure
