Common Grant Application

1. Cover Letter: This is a one page, business-style letter, that addresses the specific priorities of the funding source and includes the amount requested.

2. Summary of Applicant Organization: This is a fill-in-the-blank form that gives the funding source a one page overview of the program.

3. Narrative: This generally shouldn’t exceed three pages. It is important to include all of the following information in a concise format.

     A. Agency Information:
          • Mission statement, brief statement of the school or department’s goals or objectives.
          • Brief summary of school or department history
          • Description of current accomplishments
    

     B. Purpose of Grant:
          • A statement of the need or issue to be addressed, description of the constituency that will be served by the program (including number served), target population, and a description of how the constituency will benefit from the program.
          • Goals or objectives for the program
          • A description of the activities or actions that will allow the school or department to accomplish these goals. This is the program plan.
          • A specific timetable for implementation of the program
          • Partnerships or other organizations that will participate in the program (if any).
          • Describe how the program will be funded after the grant period or what actions are being taken in order to find further funding.
    

     C. Evaluation:
           • Define what will qualify as success for the program. What are the expected results?
           • Specifically, how will success/results be measured? 
           • How will the projects results be used or disseminated, or what will be done with the information gathered in the evaluation process?

4. Attachments:
           • List of Board of Education Members
           • List of names and qualifications of key staff for the program
           • Most recent financial statements for the school, department, or program.
           • School or department budget
           • Annual Report
           • Program budget (Budget Template)
           • IRS Tax certification letter
           • List of major contributors to school, department or program (if applicable)
           • List of volunteer involvement and in-kind contributions (if applicable)
